YOUTH REENTRY & COMMUNITY REINTEGRATION · PROPOSED PROGRAM

Every next chapter
deserves a pathway.

Sounds of Tomorrow’s proposed Juvenile Justice Education, Workforce Development, and Community Reintegration Program would connect justice-involved youth and young adults with education, mentoring, career readiness, and family support.

Program in development. This page describes the proposed service model. Launch timing, eligibility, funding, staffing, partner arrangements, and enrollment availability are to be confirmed.

PURPOSE & PARTICIPANTS

Build stability.
Restore connection.
Move forward.

The proposed program is designed for young people involved with juvenile probation, electronic monitoring, detention, or residential placement, including those returning to families, schools, and community life.

Support would begin with a young person’s strengths and goals. A trauma-informed, culturally responsive approach would connect personal growth with practical educational and employment pathways.

A COORDINATED PATH FORWARD

One plan. Connected support.

Individual mentoring, case management, structured group activities, and family engagement would work together to strengthen protective factors, support stability, and reduce further justice involvement.

THE PROPOSED SERVICE MODEL

Support that reaches
across a young person’s life.

Six connected areas would shape each participant’s plan, with services matched to assessed needs, staff qualifications, and available resources.

01 / EDUCATION

Reconnect with learning.

School engagement, tutoring, diploma or GED/HSE preparation, school reentry advocacy, and postsecondary planning. Coordination with school personnel and Child Study Teams would support individual educational goals.

02 / WORKFORCE

Prepare for meaningful work.

Career exploration, résumés, interviews, financial literacy, workplace communication, and job-search support. Connections to employers, vocational training, apprenticeships, and certification pathways would depend on eligibility and availability.

03 / MENTORSHIP

Build skills for everyday life.

Weekly individual mentoring and a proposed 12-week youth-development curriculum would focus on coping, emotional regulation, conflict resolution, decision-making, accountability, and leadership.

04 / FAMILY ENGAGEMENT

Strengthen the support around you.

Parent mentoring, family communication workshops, relationship-building activities, and reintegration planning. Families would help shape service goals and connect with appropriate community resources.

05 / COMMUNITY REINTEGRATION

Plan a supported return.

Individualized reentry planning, school reenrollment, workforce transition support, and coordination with authorized probation or reentry professionals. Plans would address supervision requirements, including electronic monitoring when applicable.

06 / COORDINATED SUPPORT

Connect the pieces.

Individual case management would help identify transportation, housing, food, identification-document, and behavioral-health resource needs, with referrals and coordination through appropriate providers.

Coping and behavioral skill development would be facilitated within staff qualifications. Clinical behavioral-health needs would be coordinated with appropriately qualified providers.

ANTICIPATED REFERRAL SOURCES

A shared commitment
to the next chapter.

The program would seek collaborative referral relationships with families, educational institutions, juvenile justice agencies, and community organizations. These are anticipated referral pathways; formal program partnerships are to be established.

REFERRAL, INTAKE & SERVICE PLANNING

A clear process.
An individualized plan.

The planned intake process would identify strengths, barriers, support needs, and goals through collaboration with the participant and family.

01 / REFERRAL & SCREENING

Begin with a conversation.

Staff would review referral information, discuss preliminary eligibility, and explain the program to the young person and their parent or guardian, as appropriate.

02 / INDIVIDUAL ASSESSMENT

Understand the whole picture.

A qualified case manager or social worker would assess education, employment interests, supervision requirements, emotional needs, family supports, basic needs, strengths, and protective factors.

03 / SERVICE PLANNING

Set goals together.

The participant, family, and staff would develop an Individualized Service Plan with measurable education, employment, personal-development, and reintegration goals.

04 / ENROLLMENT & COORDINATION

Connect with the right support.

Following eligibility and participation confirmation, a case manager or mentor would coordinate the educational, workforce, family, and supportive services identified in the plan.

PROGRESS & CONTINUED SUPPORT

Progress matters.
So does what comes next.

Ongoing progress notes, attendance tracking, monthly goal reviews, and youth and family feedback would help staff adjust support throughout participation.

Evaluation would track completion, educational engagement, workforce readiness, coping and decision-making skills, family communication, and connections with community resources.

Performance targets are preliminary planning goals and would be refined using baseline data, program capacity, participant needs, and funding requirements.

PLANNED FOLLOW-UP / 30 · 60 · 90 DAYS

Keep the pathway connected.

After the proposed 12-week curriculum, participants would receive an individualized transition plan. Follow-up contacts at 30, 60, and 90 days would review progress, identify new barriers, and reconnect participants with support as needed.
